The Crestwood Medical Group - Cardiology - Whitesport Drive Team (Congestive Heart Failure & Coumadin) is a dedicated group of nurse practitioners and cardiac nurses who work in conjunction with your cardiologist and primary care provider. We also work closely with other specialties such as nephrology and pulmonology. We strive to keep you and members of your care team informed.
Our goal is to prevent heart failure exacerbations resulting in hospitalizations and cardiac disease progression while improving patient's quality of life.
In the Crestwood Medical Group - Cardiology - Whitesport Drive, our mission is to:
- optimally treat all patients with personalized guideline directed medical therapy
- provide ongoing assessment and diagnostics to monitor for disease progression
- provide patient education to help monitor and mitigate associated disease symptoms
- promote wellness through lifestyle and diet education and as well as cardiac rehab when indicated
- address underlying or precipitating factors that contribute to the development and progression of heart failure
- discuss advanced therapies as indicated
